Description
RESPONSIBILITIES
Personal care, grooming, and activities of daily living
Assist with daily treatments
Answer call lights and resident requests as directed by supervisor
Effectively communicates and/or documents any information regarding residents
Able to perform all duties as outlined on Personal Care Assistant and Medication Aide job descriptions on evening shifts.
Replace aides if call-in occurs
Assist with medication administrations as needed PRN
Assist daily with dressing, bathing, personal grooming and hygiene, ambulation, dining, and bedside care for temporary illness, as outlined in residents' Individualized Care Plan
Assist with toileting as scheduled or needed
Assist with incontinent care, ensuring all residents are clean and dry
Monitor and report any change in residents bowel or bladder patterns
Monitor the amount of liquids resident takes and encourages fluids
Ensure residents attend and consume meals and snacks
Monitor residents on special diets
Assist residents to and from recreational activities
Engage in conversation with residents, providing comfort, mental stimulation, and companionship
Monitor and report any changes in normal routines
Monitor resident where-abouts throughout the day
Provide opportunities for residents to maintain independence
Complete charts DAILY
Respond to security system signals on a timely basis
Health and Welfare
Read the Daily Log at the beginning of each shift
Monitor daily charting for accuracy and completion, inform supervisor of problems
Monitor overall health and well being of residents; i.e.: reports changes of condition and changes in sleep patterns
Assists residents in preparing for outings and appointments
Environment & Hospitality & Resident Dignity
Manage residents' personal items, ensure they are properly marked, make sure clothing is neat and in good repair. Store items when resident is sleeping
Make sure residents' rooms are neat and tidy and that bedding is fresh and clean
Respects residents' dignity by closing doors when changing and knocking before entering
Maintain residents' dignity by only speaking in residents' predominant language
Use redirection and diversion techniques when residents are agitated
Patient and understanding when residents are slow and/or confused in tasks. Do not rush residents
Talk to residents while providing care
Treat residents with respect and dignity
Performs any other duties (within the scope of job classification and employee’s skills and abilities) as defined by supervisor.
Requirements
QUALIFICATIONS
Health screening or physical exam within six (6) months prior to employment which shows good health; free from communicable disease and disabilities which would adversely affect persons to be served; physically, mentally, and occupationally capable of performing assigned tasks; negative skin TB test
Experience in care of elderly adults, CNA/NA preferred
Current standard first aid training
Clear criminal record and fingerprint clearance
Must possess the ability to effectively read, write, and communicate in English
Show initiative when completing job duties
Is flexible in meeting the needs of the facility and residents
Follow rules, regulations, policies, and procedures
Cooperate with co-workers and supervisors
Maintain good listening skills and verbally communicates in a polite, professional manner
Create an atmosphere of warmth, personal interest and positive emphasis
Maintains a neat and clean appearance and follows established dress code
Presents a professional image at all times
Must be able to lift 50 pounds
